Nurse Practitioner (NP) - SNF Rounds | Flexible Schedule | Fall River, MA

We are hiring a full-time Nurse Practitioner (NP) to join a physician-led team providing care to patients in Skilled Nursing Facilities (SNFs) in the Fall River, MA area.

This is a great opportunity for an NP who wants flexibility, autonomy, and strong team support-with no on-call.

Position Details

• Full-Time: 30-40 hours/week
• Schedule: 4 or 5 days/week
• Flexible start/end times
• Days can be shifted (including weekends if needed)
• Ability to step away during the day for family responsibilities
• No on-call
  
  

Responsibilities

• Conduct patient rounds in local SNFs
• Manage treatment plans and ongoing care
• Collaborate with facility medical directors and NP team
• Coordinate with nursing, therapy, pharmacy, case management, and administration
• Support safe transitions and reduce hospital readmissions
  
  

Requirements

• Active Massachusetts NP license
• DEA registration
• Massachusetts Controlled Substances Registration (MCSR)
• Strong communication, organization, and clinical judgment
• Ability to work independently and as part of a team
  
  

Preferred (not required): Portuguese fluency
