Dear New York Nurse Practitioners,
We are writing to inform you of an innovative on-line diabetes education and support research study that may be of interest to you and your patients. Diabetes LIVE is an RO1 Randomized Controlled trial that is federally funded by NHLBI for 5 years and approved by NYUMC. The aim of this study is to examine if a web-based self-management and social support intervention will improve health behaviors and metabolic outcomes in patients who have Type 2 Diabetes. NO MEDICAL care is provided and this DOES NOT replace diabetes education received from participant healthcare practices. We have attached a brochure and provider pocket card summarizing the program and here is a link to a demo and explanation of the LIVE (Virtual) and Web environment.
